Rosters - Where is "add service record" option?

I must be overlooking this, but where is the link/page to "add service record" to a member on the roster? Thanks!

Disregard, was a special permission unticked in "moderator" side of permissions. Thanks!

Experiencing another issue. There is no option to select a service record "type", nor am I finding a location to edit these fields. Thanks
