Tickets Fixed 1.4.8 Cannot submit tickets: Mysqli prepare error: Unknown column 'user_name' in 'field list'

Backtrace attached.

This occurs whether attempting to submit a ticket as a user or guest.

Tickets 1.4.8 was installed as a clean install, not an upgrade.

Solved by manually recreating the xf_nixfifty_ticket table using the query in install.php; a few other tables were not created properly as well.
