These are a couple of items that I've encountered that make filling out tickets less easy as they can be and hope you'll be able to answer or maybe even address these preferences.
1. Informational fields should appear at the top of the form before the comment field.
Regardless of whether users are registered or not, the name and email address of the user should be presented first above all other custom fields, which is what users typically expect. In addition, custom fields should appear first above the support comment textbox so the user doesn't try to categorize and explain details about the problem to only see the custom fields after and realize all of this wasn't necessary (or incorrectly described.)
2. Title Field = Support Type and not shown.
Another nice option would be to remove the subject line and replace with the category name. For example, I don't want even more clutter in my inbox or to have users do more work just to send a ticket. If it is "tech support" or "sales" or "feedback" I'd like to be able to see that alone in the subject header so I know what it is and how to prioritize.
